TERMS & CONDITIONS OF TREATMENT AND CARE

The below information describes the Terms and Conditions under which the Veterinarians, Clinical Staff, and Support Staff of Care Animal Center will provide care, treatment, and services to your pet(s).

*In accordance with State of Alabama Law, all dogs and cats must be current on rabies vaccination. Proof of vaccination must be presented at time of check-in or vaccination will be administered.

*To help prevent the spread of infectious diseases, hospitalized and boarded animals must be current on all required vaccinations. Proof of compliance with all required vaccinations must be presented at time of check-in or vaccinations will be given.

*Every effort will be made by Care Animal Center to achieve a successful outcome and to provide for all possible safety in hospital care and handling; however, success cannot be guaranteed.

*Care Animal Center will provide you with prices for care, services, and products upon request.

*Care Animal Center will provide you with estimates for surgical procedures and other advanced care services (such as canine dental procedures or chemotherapy) as requested by you.

*You consent to and authorize Care Animal Center to receive, prescribe for, treat, or perform surgery upon your pet(s).

*You consent and agree to receiving emails from Care Animal Center via any email address you provide to us. These emails may be in the form of appointment reminders, pet prescription refills, general pet health tips, or special pricing or discount for services at Care Animal Center.

*You consent and agree that, in order for us to service your account and your pet(s) appropriately or to collect monies you may owe, Care Animal Center and/or our agents and associates may contact you by telephone at any telephone number associated with your account, including wireless telephone numbers, which could result in charges to you.

*You consent and agree (by providing your phone number) to receive SMS text messages from Care Animal Center for appointment reminders, occasional marketing messages, and general two-way communication with our Veterinarians and Staff. Msg frequency varies. Msg&Data rates may apply. Reply HELP for support. Reply STOP to any message at any time to opt out; you may also contact us at 334-794-6333.

*You agree to pay all fees for services rendered at the time the pet(s) are discharged from the hospital or as agreed to prior to treatment.

*You agree to pay for the reasonable cost of collection in the event that collection efforts become necessary. You accept the fees charged as a legal and lawful debt and agree to pay said fees including collection agency fees, attorney fees and/or court costs if such become necessary.

*You understand and agree that Urgent/Emergent and After Hours Services are provided as necessary based on the judgement of the Veterinarian on Call.

*You understand and agree that Care Animal Center will not share your personal information or your pet’s medical information with any outside party or third party for marketing purposes.

*You understand that your personal information and your pet’s medical information will be handled at all times in accordance with Care Animal Center’s Privacy Policy which can be viewed at www.careanimal.net.
